Automatic contouring

The automatic contouring algorithm in the ORMIR_XCT package generates the periosteal mask of the distal and proximal bones of the wrist joint as well as a joint masking combining the distal and proximal bone masks. Either the gray value image can be imputted or a gray value image and rough periosteal masks of the distal and proximal bones. The latter is a variant for when the distal and proximal bones are too close to each other (i.e. narrow joint space width) to distinguish.
